The delivery of an enhanced Primary Care Team is part of a new approach to Mental Health Primary Care services in Mid Essex. This team will be based in Mid Essex Locality, working within General Practice to triage, manage, and follow up with patients registered with GPs in the area.
This innovative approach introduces new roles to complement traditional General Practice roles, aiming to manage more patients at the point of contact. The team’s skill mix is based on a local population needs assessment and includes Clinical Pharmacists, Advanced Nurse Practitioners, Physicians Assistants, and Community Psychiatric Nurses.
This is an exciting role for a Primary Care Mental Health Practitioner to lead and shape community mental health services. The post is hosted by Essex Partnership University NHS Foundation Trust (EPUT) with day-to-day management within Primary Care.
As a highly skilled and motivated Primary Care Mental Health Practitioner, you will be part of the Enhanced Primary Care team, working autonomously to provide evidence-based psychological and non-psychological treatments for common mental health disorders.
The role involves working proactively in the locality, offering interventions over up to 6 months, including mental health monitoring, guided self-help, brief interventions, medication initiation, and depot injections for discharged patients. The team collaborates with wider networks to ensure early intervention and proactive care at the primary care level.
