Description
We are seeking a dedicated Lead Support Worker for a permanent, full-time position (37.5 hours per week), working on a rota basis.
Pay and Benefits
£13.21 per hour (equivalent to £25,838 per annum) plus benefits including a Health Cash Plan.

Join our team to support customers with complex mental health needs, helping them develop skills for independent living. Your work will make a positive impact each day, helping customers achieve their goals.
Key Responsibilities
1. Develop and implement person-centred support and risk management plans with customers.
2. Assist with daily activities such as cooking, shopping, budgeting, maintaining tenancies, and supporting moves to independent accommodation.
3. Collaborate with health and training professionals to support positive health and employment outcomes.
4. Conduct risk assessments, support planning, and regular reviews.
5. Build rapport through informal chats and support collaborative working with colleagues.
Candidate Requirements
* Passion for supporting individuals to live their best lives with attention to detail.
* Experience in creating support plans and supporting colleagues in their implementation.
* Experience in coordinating customer referrals and assessments.
* Ability to work independently, remain calm under pressure, and demonstrate resilience.
* Must have a vehicle insured for business use; mileage expenses will be reimbursed.

Team and Location
You will join our Hampshire West team, based in Lymington, with some travel required for meetings and outreach activities. Our team is passionate about our work and supportive of each other.
Working Patterns
* Rolling rota including shifts from 9am-5pm, 10am-6pm, 11am-7pm, with some weekends and bank holidays.
* Flexibility to adapt start and finish times to meet customer needs.
* Regular team meetings and training, with occasional lone working.
* Proficiency in technology for support planning, online learning, and collaboration.
* Enhanced DBS check required, paid for by us.
