Job Summary
Working Full time: 37.5 hours per week
We are looking for a Rehab Assistant to work within the Rapid Response Team, providing an early supported discharge service to patients from The Countess of Chester Hospital & Ellesmere Port Hospital.
Shift work over 7 days between 8am and 9pm, lone working across West Cheshire. A full driving licence and car are required.
Main duties of the job
* Assist patients with their activities of daily living, providing resettlement, rehabilitation and care in their own home, delivered in accordance with the patient’s Care Plan.
* Support therapists and nurses with their caseload, manage a caseload of your own under the supervision of qualified staff and Senior Community Support Workers, and perform general administrative and housekeeping tasks as required.
* Demonstrate excellent interpersonal skills, be self‑motivated, empathetic and develop practical skills. Provide a holistic view of patient care focused on patient safety and independence in the home.
* The Rapid Response Team provides a service 7 days per week, including bank holidays and evening shifts.

Qualifications Essential
* NVQ Level 3 or equivalent, or evidence of working towards.
* ECDL or CLAIT qualification, or ability to demonstrate use of basic Microsoft packages (Word, Excel, PowerPoint).
Experience Essential
* Previous working experience or volunteering with people in a healthcare/social care setting or as a carer.
* Demonstrated empathy and understanding.
* Good organisational and time‑management skills.
