Practice Clinical Pharmacist
We are seeking a motivated and experienced pharmacist to work directly with multidisciplinary teams to improve clinical effectiveness and person‑centred outcomes. The successful candidate will play a key role in medicines optimisation, clinical medication reviews and the safe and effective use of medicines across the practice population.
Responsibilities
* Conduct structured medication reviews (SMRs) for patients with complex polypharmacy, frailty or long‑term conditions.
* Manage and monitor long‑term conditions under appropriate clinical supervision.
* Provide face‑to‑face, telephone and digital consultations.
* Support safe deprescribing and medicines reconciliation following discharge or clinic visits.
* Implement national and local guidelines to ensure evidence‑based prescribing.
* Undertake clinical audits and quality improvement initiatives.
* Identify and manage medicines‑related risks, safety issues and MHRA alerts.
* Support antimicrobial stewardship initiatives.
* Review repeat prescriptions, ensuring accuracy, appropriateness and compliance.
* Oversee repeat prescribing processes and medication synchronisation.
* Support medicines optimisation, cost‑effective prescribing and formulary adherence.
* Assist with shared‑care agreements and high‑risk medication monitoring.
Qualifications
* Registered Pharmacist with the General Pharmaceutical Council (GPhC).
* Evidence of continuing professional development (CPD).
* Minimum of 2 years post‑qualification experience.
* In‑depth therapeutic and clinical knowledge and evidence‑based healthcare principles.
* Understanding of primary care prescribing and rational prescribing strategies.
Desirable Qualifications
* Knowledge of EMIS Web.
* Experience working in general practice.
* Independent prescriber or working towards independent prescribing qualification.
* Clinical diploma.
Experience

Contract: Fixed term (6 months)
