Job Summary

Join our team of GPs and ANPs in the New Models of Care programme for Wellingborough and District. Work within our practices in the Wellingborough neighbourhood to deliver high‑quality, same‑day care to our patients. We’re looking for GPs to join our Neighbourhood Access Service, part of the innovative New Models Programme. You’ll work autonomously and as part of a multidisciplinary team within primary care to provide expert assessment, diagnosis, treatment, and evaluation for patients presenting with undifferentiated primary care presentations. This is your chance to shape integrated neighbourhood services and make a real difference to patient outcomes.

Main Duties

As a GP in the New Models of care pilot, you will provide autonomous care for our patients presenting with undifferentiated presentations mainly in the same day settings. You will demonstrate safe clinical decision making and provide expert care by undertaking initial history taking, clinical assessment, diagnosis, treatment and evaluation of care, including appropriate follow‑ups. Working with other GPs and nursing team in the neighbourhood Practices and Hub you will continually review and evaluate services and work with the management team in implantation of change.


About Us

The Neighbourhood of Wellingborough, consisting of 8 GP Practices are seeking to develop an innovative new model of care. You will be working with a team of GPs and other specialist ANPs to deliver same day care to the Wellingborough population.

Role Summary

As a GP in the New Models of care pilot, you will provide autonomous care for our patients presenting with undifferentiated presentations mainly in the same day settings. You will demonstrate safe clinical decision making and provide expert care by undertaking initial history taking, clinical assessment, diagnosis, treatment and evaluation of care, including appropriate follow‑ups. Working with other GPs and nursing team in the neighbourhood Practices and Hub you will continually review and evaluate services and work with the management team in implantation of change.


Overview of Responsibilities – Clinical

* Provide clinical care to our patients. This will include reducing the demand on the core neighbourhood GP practices by the assessment and manage of patients and in the community by means of telephone triage and clinic appointments.
* Use advanced skills and expert knowledge to assess the physical and psycho social needs of patients with undifferentiated primary care conditions including minor illness and instigating and providing in partnership with other care providers, therapeutic treatments based on best available evidence to improve health outcomes.
* Use advanced skills and expert knowledge to make comprehensive and focussed assessments, independently and proactively managing care of the patients.
* Promote patient independence and self‑care where appropriate.
* Support and work cohesively in a team with other GPs/ANPs and ACPs working in the new model of care settings, including providing oversight and mentorship for more junior colleagues.
* Review pathology results to ensure appropriate treatment and care.

Support and lead the practices to the delivery of the Quality and Outcomes Framework and NHS England/ICB/Federation Contracts.


Regulatory

* Respond to complaints and clinical incidents/significant events as appropriate to ensure robust systems are in place for improving practice through joint learning.

Maintain accurate and timely clinical records in line with professional and legal standards.

* Undertake at least one clinical audit a year. Evaluate data and present findings to the Practice Clinical team. Implement changes where appropriate.
* Adhere to the employing practice Policies and Procedures including but not exclusively Infection Control, Health and Safety, and safeguarding. Ensure all mandatory training is completed with a timely manner.
* All staff must be familiar with and actively always promote and work within the spirit of the practices Equal Opportunities Policy.

* Maintain own personal/professional development plan and portfolio thereby meeting the requirements for revalidation through appraisal of own performance and discuss development needs with mentor, supervisors and managers, on a regular basis.
* Adhere to the annual appraisal process and revalidation requirements.
* Work at all times within the GMC Code of Professional Conduct.
* Provide Mentorship and training of new recruits or students as and when required.
* Participate in the Practice Performance Review process.


Person Specification – Qualifications

* CCT qualification
* On the performers list register for Northamptonshire
* Relevant first level degree
* Extensive clinical experience in primary nursing
* Evidence of continuing professional development
* Teaching and assessing
* Cultural awareness
* Used to prioritising varied workloads to meet deadlines
* Used to dealing with all levels of staff
* Evidence of ability to teach/disseminate knowledge and skills.


Person Specification – Desirable Qualifications

* Relevant Master level degree
* Management qualification/experience
* Post registration qualification in relevant to area such as minor illness/injury, women's health/mental health/paediatrics
* Service development role
* Teaching qualification
* Recent attendance on a leadership skills development programme.

* In depth knowledge base in the management of patients with chronic illness and minor illness
* Ability to maintain and monitor high standards of evidence based care
* Ability to direct and co‑ordinate care working autonomously and collaboratively
* Understanding of clinical governance issues
* Evidence of improving the patients experience.

* Evidence of leading a service project at a practice level
* Undertaking research or involvement in clinical research programmes
* Experience in writing clinical policies and protocols
* Counselling experience.
