The Clinical Team Lead is a senior clinical role within the service, providingprofessional leadership, supervision, and support to the Home Visiting Serviceclinical team. The post-holder will work closely with the Clinical Directors to ensure the safe, effective and consistentdelivery of the service.
The role would be suitable for an advanced nurse practitioner, senior practice nurse, consultant nurse, senior paramedic, or advanced clinical practitioner. The candidate should have line management experience.
Main duties of the job
Clinical Leadership and Support
Provide line management and clinical guidance to Home Visiting Serviceclinicians, supporting complex decision-making and patientmanagement.
Support coordinators and clinicians with clinical triage and referrals,ensuring appropriate patient selection and escalation.
Provide triage cover when required to maintain service continuity.
Review and appropriately challenge referrals made outside the HVSinclusion criteria, ensuring safe, appropriate patient pathways.
Offer advice and support to coordinators when uncertainaboutaccepting referrals.
Provide case-based learning and feedback following complex cases,incidents or audit findings.
Support the integration of clinical governance principles into dailypractice.Participate in and lead clinical audits to ensure safety, quality andeffectiveness of the HVS.
Provide clinical supervision and mentoring for both substantive andlocum clinicians.
Identify training needs and develop learning events to supportprofessional development across the team.

The Home Visiting Service (HVS) plays a vital role in supporting practices by managing acute presentations and providing continuity of care for frail and housebound patients, helping to reduce unnecessary hospital admissions.
Job responsibilities
The Clinical Team Lead Paramedic will:
Provide line management, senior clinical leadership, mentoring, and support to Paramedics and Coordinators within the HVS team.
Offer clinical oversight and advice to support safe triage, assessment, and decision-making.
Collaborate with the and Clinical Educational Lead to ensure clinical quality, governance and continuous service improvement.
Promote professional development within the Paramedic team, leading on clinical learning, audit, and best practice.
Contribute to strategic service development and innovation across the Sutton PCNs in partnership with Clinical Directors/ Board Directors of Sutton PCNs.

Work closely with the and Clinical Operations Lead to align clinicaldelivery with operational processes.
Contribute clinical expertise to service development, quality assurance,and performance monitoring.
Support the Proactive care team by undertaking assessments ofindividuals referred to the service as required and develop care planswith the patients/family / carers as appropriate
Ensure Universal care Plans are completed and uploaded for theseindividuals
Support the with implementation of clinical governance frameworks andprotocols
Collaborate with Clinical Directors, Practice Leads, and ARRS roles tosupport integrated care delivery.
Participate in multidisciplinary team meetings, promoting collaborationand shared learning.
Liaise effectively with GPs, community teams, urgent care providers,and social care partners to ensure seamless patient care.
Clinical Practice
Assess, diagnose, and manage patients at home or in primary caresettings, using advanced clinical reasoning and evidence-based practice.
Provide continuity of care through follow-up and safety-netting whererequired.
Prescribe or recommend medication where appropriate and within scopeof practice.
Contribute to reducing unnecessary hospital admissions and improvingpatient outcomes.
Maintain high-quality clinical documentation and data integrity.Administrative and Governance Support
Line manage the Home Visiting Service Clinical team.
Oversee completion and accuracy of end-of-day timesheets, task lists,and activity reporting.
Support clinical audit, data collection, and reporting requirements..
Contribute to service reviews, quality improvement projects, and CQCreadiness.
Uphold information governance, confidentiality, and data protectionstandards.
Person Specification
Experience
* Significant post-registration experience at least 5 years in primary, urgent, or community care.
* Experience working within a Primary Care Network or multi-professional team.
* Experience supporting service transformation or clinical innovation.
Qualifications
* Registered healthcare professional with current UK registration (e.g.
* NMC, HCPC, or equivalent).
* Significant post-registration experience (normally 5 years or more) in
* primary care, urgent care, community care, or a relevant setting.
* Experience of clinical leadership and line management
* Demonstrated ability to practise autonomously and manage complex
* clinical presentations.
* Strong communication, leadership, and problem-solving skills.
* Sound understanding of clinical governance, audit, and quality
* improvement.
* Ability to work across sites, including undertaking home visits.
* Full UK driving licence and access to a vehicl
* Advanced Clinical Practice qualification or working towards one.
* Independent prescribing qualification (or working towards).
* Experience working within a Primary Care Network or multidisciplinary
* team.
* Understanding of population health management and integrated care
