Job overview

An exciting opportunity has arisen for a motivated, enthusiastic qualified District Nurse to become the Team leader of the Lightwood Central service.

The post holder will lead, manage and supervise a Community Nursing Team, facilitating research and evidence-based practice. Lead the assessment process and maintain high quality effective nursing care in the community as part of the wider integrated care team. Other duties will be to ensure service delivery is in line with the Core Offer and the Standard Operating Procedures of the Community Nursing Service

This large dynamic team is part of the wider Integrated Community Care structure and works collaboratively in order to deliver high quality, individualised patient care at home.

Main duties of the job

Determine and prioritise the health needs of the Community Nursing caseload.

To be aware of the health needs of the practice population and contribute to the public health agenda for the local community, so resources can be targeted appropriately.

Responsibility for the day-to-day management of the District Nursing Team.

Lead, manage and co-ordinate the assessment, planning, implementation and evaluation of care programmes, reflecting the needs of individual patients / carers and families.

To be responsible for the performance of the District Nursing Team and individuals in it.

Promote the principle of self-care and patient empowerment.

Working in partnership with other agencies, act as care manager for identified patients in order to prevent inappropriate hospital admission.

Manage resources effectively and advise on skill mix requirements for the team. In collaboration with line manager and manage team budgets.

Positively participate in and promote Clinical Governance, thus ensuring the highest quality of practice is maintained, within the service. Demonstrate the use of evidence based practice and participation in clinical audit.

Lead and support the team in providing care to patients in their own home.

Qualifications

Essential criteria

1. Community Specialist Practitioner (District Nurse) degree.
2. Registered Nurse, first level with a valid NMC registration
3. Nurse prescriber (V150)
4. Evidence of ongoing continuous professional development.
5. Mentorship/Preceptorship Chronic disease management
6. To have completed the national practice assessor training or equivalent teaching and assessing qualification for those with ENB998.
7. Clinical Practice Educator or willingness to working towards

Desirable criteria

8. Non Medical Prescriber (V300)
9. Clinical Practice Educator Qualification

Special Skills/Aptitudes

Essential criteria

10. Knowledge of local/ national policies
11. Awareness of clinical governance issues
12. Knowledge of implementation of evidence based practice.
13. Demonstrate an understanding and good working knowledge of IT
14. Proven team working, management and leadership skills

38. Due to the requirement to undertake visits and transport non-portable equipment, the post holder is required to be a car driver with the use of a car with a valid UK driving licence.
