Overview
The Clinical Practitioner in Urgent Community Response will act as a generalist “expert” and exercise their clinical expertise in the long‑term conditions (LTC’s) including frailty, levels of judgement, discretion and decision making in clinical care to deliver the Urgent Community Response Model.
Responsibilities
* Work closely with the wider multi‑disciplinary workforce in primary, community and secondary care, to ensure patients receive appropriate investigation, intervention and treatment with minimal avoidable delays, while improving the quality of care in the right setting.
* Work autonomously assessing individual patient needs, initiating investigations and launching appropriate holistic, evidence‑based treatment with development of a personalised clinical management plan, including independent prescribing.
* Ensure the patient is placed at the centre of all decisions, reinforcing the concept of a streamlined patient journey to improve the quality of care.
* All staff, workers, volunteers, students and individuals undertaking work experience or shadowing, irrespective of the role they specifically undertake, are required to adhere to BCHC’s vision, values and professional standards. This also involves working with and alongside colleagues and partners, demonstrating a duty of candour (i.e., honesty and straightforwardness), openness and accountability to achieve high quality and the best possible care outcomes for patients, service users and the local community.
* Develop staff knowledge and skills to promote equality and diversity and address inequalities, both in employment and service delivery. Ensure specific equality objectives are included in PDRs.
* Develop a culture that ensures that the standards of Improving Working Lives and Investors in People are achieved and maintained for all staff and that staff’s perception of their working lives is measured and improvements made.
* Make recommendations and changes to clinical practice and participate in implementation where appropriate.
* Demonstrate a sound understanding of Clinical Governance and Risk Management and apply it to the work situation.
* Lead and participate in undertaking surveys and audits.
* Actively participate in clinical supervision and reflective practice with all members of the multi‑disciplinary team and on an individual basis.
* Ensure staff compliance with all mandatory training.
