How You’ll Make a Difference

Working within an integrated mental health team as an Approved Mental Health Practitioner, you will provide effective interventions for people with severe and enduring mental health needs and those in their networks. Your work helps enable them to meet daily health, social care and wellbeing needs in line with their personal recovery goals.


What You Will Be Doing

* Under the overarching framework of "Your Team, Your Conversation, Your Plan", you will work with agencies and services within an integrated health and social care model as a key worker for service users. In line with best practice legislation, policy and guidance, you will promote users’ wellbeing and dignity.
* Deliver genuine people‑centred, strengths‑based support for service users, implementing, reviewing and monitoring actions and outcomes in line with their individual care plan, making amendments as needs change and evolve.
* Regularly work with people who may require social care services, their families and carers to ensure independence is maintained as much as possible and outcomes are clear, monitored and reviewed.
* Prepare social circumstances reports for the Mental Health Review tribunal and attend tribunal hearings.
* Maintain up‑to‑date client records and client record systems.
* Possibly undertake student supervision for social work students.


What We Need From You

* Hold a degree in social work, be registered to practice, hold current registration with Social Work England and be committed to CPD.
* Substantial post‑qualification experience, ideally in mental health services, with a detailed understanding of relevant legal frameworks such as the Mental Health Act, Mental Capacity Act and safeguarding.
* Demonstrated understanding of the Recovery principles and the role of secondary mental health services in care delivery; experience undertaking carers’ assessments, risk assessment and developing risk‑management strategies.
* Strong verbal and written communication skills, including active listening, able to engage effectively at all levels and assertively when required.
* Preferred: post‑registration qualifications such as BIA, CBT, AMHP and Brief Interventions; familiarity with the "Your Team, Your Conversation, Your Plan" process (training will be provided).


What You Need To Know

* Salaries pro‑rated to hours worked.
* Enhanced Disclosure and Barring Service (DBS) check is required for this post.
* Travel throughout the authority and surrounding area; must have a full, valid driving licence and regular vehicle access or alternative transport.
