As a Move on Officer,youllwork with people within the city who have been living in Support Housing and are now ready to take that step into independent living. You will do this by working with Supported Housing providers and the local authority to help deliver a move-on plan for people who are ready to transition into sustainable independent living.
Job requirements
Conduct assessments, risk assessments, inductions, and support plans collaboratively, reviewing plans monthly.
Develop and deliver person-centred, trauma-informed move-on plans tailored to individual needs, strengths, and goals.
Support financial readiness and explore suitable housing options for successful move-on.
