An exciting opportunity has arisen to work within the young people with dementia team. We are looking for 2 Community nurses to join the team. The role involves working closely with service users and families completing cognitive assessments, requesting scans, diagnosing and holding a case load. Further involves supporting service users and their families following a diagnosis of young onset dementia. This is a community post and covers Northamptonshire which can involve a lot of driving to different areas. Car driver essential /with access to own transport.

Working for the young people with dementia team- which is a countywide service with people under the age of 65- covering Northamptonshire. This post involves initial assessments holding a case load working autonomously and working closely with the MDT .


Main duties of the job

The post will involve close MDT working and working autonomously around the community.

Excellent communication skills are required and a good knowledge base of dementia and generic mental health conditions.

Good assessment skills will be required, must be able to write comprehensive reports. Good interagency collaborative skills and the ability to focus on empowering service users to live well with dementia.

* Band 6- community experience, knowledge and working with people with dementia.
* Car driver essential and access to own vehicle.
* At least 4 years qualified experience
* Dementia knowledge particularly with young people


Presentation

* Power point presentation
* Demonstrating knowledge of young onset dementia
* Good communication skills and the ability to demonstrate knowledge.
