Job Summary
We have an exciting opportunity within the Community Ready and Reablement Unit for a band 5 registered nurse. This unit is part of the Trust's 'One step closer to home' philosophy. We are looking to appoint an enthusiastic and hard-working Band 5 Registered Nurse to work with the multi-disciplinary team on the Community Ready Unit. This new post is an exciting opportunity to support patients awaiting discharge.
The Community Ready Unit offers a relaxed environment for patients who are community ready and may be awaiting various community support. The successful candidate will work as part of our Transfer of Care Hub to support patients who are community ready to continue their rehabilitation potential. This unit opened in January 2024 and we are continually striving to improve the quality of our service. The post holder would be expected to work a rotation of days, nights, and weekends to meet service needs. We seek someone who is enthusiastic, creative, and positive with outstanding communication skills.
Main duties of the job
1. To be responsible and accountable for delivering safe, effective, and compassionate nursing care.
2. To work effectively within a multi-disciplinary team ensuring appropriate interventions to improve health and independence.
3. To always work in accordance with the NMC Code.
4. To supervise non-registered staff as stipulated by the NMC under delegation requirements.
5. Provide effective leadership and assist with management and organization of the unit in the absence of the Ward manager or deputy.
6. To actively support safe and timely discharge of patients to their own home or residence.
7. To comply with the trust's vision, values, and behavioral expectations.

Person Specification
Experience
* Essential: Experience in an acute hospital or community healthcare setting.
* Desirable: Recent experience as a registered nurse.
Physical Skills
* Assist patients with movement as per Trust policies.
Qualifications
* Current NMC Registration or evidence of qualification if newly qualified.
* Diploma in health-related studies or equivalent.
* Desirable: Degree in nursing or healthcare, mentorship certificate, or equivalent experience.
Skills and Knowledge
* Ability to communicate complex, sensitive information.
* Ability to work independently.
* Competent in IT (e.g., Microsoft Office).
* Evidence of Continuing Professional Development (CPD).
* Knowledge of health and social care policies and evidence-based guidelines.
* Desirable: Venepuncture and cannulation skills.
