An exciting opportunity has arisen to join the Acute Hospital Sector Discharge to Assess Teams, based within the Queen Elizabeth Hospital and Heartlands Hospital front door OPAL admission avoidance teams. The post is with Birmingham City Council as a Social Worker, Permanent, Grade 4, with a 36.5 hour per week contracted pattern and a Golden Hello payment package (see details below). The role involves working on Discharge to Assess- Acute Hospital front door OPAL Teams and requires working bank holidays and weekends on a rota system.
Due to the Pay Equity Review, the Consultation grade will be subject to formal evaluation.
Responsibilities
* Work across busy Multi-Disciplinary Teams (MDT) and may be required to support other teams within the Discharge to Assess Service, as appropriate.
* Support and strengthen MDT, fostering collaborative and effective working practices within a fast-paced environment to ensure high-quality outcomes for citizens and safe discharges.
* Facilitate social work practices within the Discharge to Assess MDT, enabling citizens to exercise choice, control, independence, and resilience with a focus on prevention and early intervention.
* Deliver person-centred practices, provide guidance and signposting, and complete assessments in accordance with the Care Act 2014.
* Carry out social work functions using relevant legislation, produce reports, and manage workloads to meet required timeframes under pressure.
* Work at Acute Hospital front door OPAL Teams (Queen Elizabeth Hospital and Heartlands Hospital).
* Be on a rota to cover weekends and Bank Holidays; the OPAL Team operates seven days a week (one weekend in every four).
Requirements and Qualifications
* Demonstrable social work experience and the ability to work positively as part of a Multi-Disciplinary Team, in partnership with other stakeholders, with an interest in and skills to work within the Discharge to Assess MDT.
* Ability to produce reports and resilience to deliver under required timescales.
* Working within the legislative framework and using relevant legislation to complete conversations and provide support and guidance; strong interpersonal and communication skills.

* Full-time employment, 36.5 hours per week, with weekends and Bank Holidays on a rota.

Remuneration, Benefits and Additional Details
* Grade 4 - £34,434 to £41,771; Consultation grade subject to formal evaluation under the Pay Equity Review.
* Golden Hello payment - £1,000 + £5,000 per annum; see details and conditions.
