Job Description – Practice Nurse Job Summary We are seeking a dedicated and enthusiastic Practice Nurse to join our supportive primary care team. This is a flexible role, offered on a full-time or part-time basis, with a negotiable salary dependent on skills and experience. The successful candidate will play a key role in delivering high-quality nursing care to our patient population, working closely with GPs and other healthcare professionals. Main Duties & Responsibilities Provide chronic disease management (e.g. asthma, COPD, diabetes, hypertension). Perform cervical cytology (smear tests), childhood and adult immunisations, and seasonal flu/COVID-19 vaccinations. Conduct wound care, dressings, and phlebotomy as required. Support patients in health promotion, lifestyle advice, and self-management of long-term conditions. Triage and manage patient presentations appropriately, escalating to GPs or senior colleagues where needed. Maintain accurate and timely clinical records in line with NMC standards. Participate in infection control, safeguarding, and clinical governance activities. Contribute to QOF, IIF, and other quality targets, supporting practice and PCN priorities. About You Registered Nurse (NMC) with experience in a primary care or community setting. Confident in chronic disease management and preventative healthcare. Competent in vaccinations, cervical screening, and phlebotomy (training can be provided if needed). Excellent communication and teamwork skills, with a patient-centred approach.
