PhyllisTuckwell provides palliative and end of life care for people living with anadvanced or terminal illness. Based in Farnham and Guildford, we serve apopulation of 550,000 across West Surrey and North East Hampshire.

This agreat time to join Phyllis Tuckwell as we develop services for our 2024-2029strategy, consolidate our Community and Living Well base at The Beacon Centrein Guildford and prepare to return to a new purpose-built Hospice in Farnham.

The rolerequires a person with excellent skills in leading and motivating a team, theywill inspire the team to work cohesively, safely and effectively and will havethe resilience and skills to provide leadership to continuously improveservices for our population. The rolewill support our vision, We want everyone to have the best possible experienceat end of life.

Hospice at Home Manager

37.5 hours per week

Salary: £63,550 WTE pluson-call allowance

Location: Farnham with travel between our sites andacross our catchment

Phyllis Tuckwell is looking to appoint a Hospice at Home Manager withexperience and expertise in clinical and nurse leadership and management, acommitment to person and family centred palliative and end of life care and tocommunity working.

The role is both internal and external facing and supports high-qualitycare and seamless operational multidisciplinary team working around the patientand family.


Main duties of the job

The Hospice at Home Manager will ensure a One Teamapproach around the patient and family.They will develop our Community Ward and will consolidate proactiveplanned work. They will supporteffective practice and learning for all Community Nurses and will work as ahighly effective member of our Clinical Leadership Team. The successful candidate will be a visible,authentic leader and will promote a learning culture, teamwork andcollaboration.

Be a registered nurse

Have strong knowledge and skills in clinicalgovernance and will value hearing and acting on patient and family experience.

Be able to collaborate and partner with teamsoutside of Phyllis Tuckwell and value and enact effective teamwork withinPhyllis Tuckwell.

REPORTS TO AND ACCOUNTABLE TO: Director of Patient Services

JOB PURPOSE:

The postholder is a member of the Clinical LeadershipTeam who drive forward excellence in palliative and end of life care andcontinuous improvement and who work collaboratively and cohesively aroundpatient and family needs. This NursingLeadership role will focus on developing our new Community Ward, and on leadingeach of the three Community Locality Teams within our Hospice at Home Service.

The postholder will inspire members of the team to providetimely, responsive care that wraps around patient and families, safely andeffectively. The role plays a key partin developing services ensuring that they are fit for the future of PhyllisTuckwell and population needs.

Key responsibilities include to:

Ensure compassionate, individualised, person centred care for people atend of life and their families, in their homes and when they are visiting oursites.

Support staff wellbeing so that people who work in Phyllis Tuckwell CommunityServices, enjoy coming to work, feel challenged and supported, are accountablefor their actions, are inclusive and treat others with dignity and respect.

Foster a seamless, One Team operational model based around the familyand patients physical and psychological needs, promoting integration, effectivecare, and efficient use of resources.

Promote responsive, evidence-based, high quality, end of life care forpatients, carers and relatives.

Deputise for the Director of Patient Services as needed and take part inthe Clinical Manager/Lead on call out ofhours rota.

Provide strong, effective clinical leadership, clinical governance andmanagement to Community Services.

Work effectively with all colleagues and the many teams within PhyllisTuckwell including all of the support teams and the Clinical Leadership Team.

Support innovation and opportunities for growth, learning, anddevelopment of roles and careers across the In-Patient Unit and community.

Manage change effectively recognising the impact on people and onservices and seeking to support cohesive teamwork and safe and effective careduring periods of change.

Contribute to the strategic development of Phyllis Tuckwell, helping totranslate our current strategy into effective operational delivery and toensure that services reflect, and are responsive to, current and future end oflife care needs.

Support partnerships, by fostering collaboration with other hospices,teams and services and hospitals to enable everyone to have the best possibleexperience at end of life, and to increase awareness of and referrals intoPhyllis Tuckwell.

SERVICE DESCRIPTION:

The Phyllis Tuckwell Community Servicecomprises:

Hospice at Home:

Services for patients living and dying at home, and support for familiesand carers includes:

Planned Proactive home-based care with nursing teams in each of threelocalities, each led by Senior Clinical Nurse Specialists. The locality teams work with the MultidisciplinaryTeam around patient needs.

For patients and families with for patients who are in an unstable phaseof illness, who have rapidly changing palliative care needs or who are in theirlast days or short weeks of life. Includes:

Community Ward

A Virtual Ward - Consultant-led medical service for patients andfamilies with heightened palliative care needs.

Living Well: site based proactive, planned palliative and supportivecare for patients who are stable or slowly deteriorating, their families andcarers.

This Hospice at Home Manager role works within theDirector of Patient Services Team of: Community Services Manager for LivingWell and Therapies, Patient and Family Support Manager, Inpatient Unit Manager,Clinical Education Manager, Deputy Director for Nursing and Quality and as partof the Clinical Leadership Team which includes the Medical Director, Consultantsin Palliative Medicine and the Clinical Administrator Manager.

This is an exciting phase for Phyllis Tuckwell, YourNew Hospice project is well underway, we will occupy a new building on ourFarnham site in 2026, our Hospice at Home services also operate from The BeaconCentre in Guildford and in our Communities, this role requires visibleleadership and travel across the catchment area.

RESPONSIBILITIES:

1.0Professional and Clinical Leadership

a.To lead, develop and manage the community multi-professional team ensuringhigh quality integrated care across service areas.

b.To foster partnership working with patients and families and integratedcare around the patient.

d.To develop the community model, to ensure robust and responsive servicedelivery.

e.To provide a visible, accessible senior clinical management presence tothe teams, monitoring professional standards and effective use ofresources.

f.To maintain own clinical knowledge and skills through clinical decisionmaking, use of clinical judgement in relation to patient safety and clinicalgovernance and via clinical work.

g.To keep under review clinical practice and nursing practice through themonitoring of quality standards, clinical supervision, clinical audit, riskmanagement, critical incident review and reflective practice.

h.To maintain oversight of caseloads, monitoring patient acuity anddependency, sill mix and staffing levels ensuring resources are utilisedeffectively, patients and family needs are prioritised according to serviceneeds, and wellbeing of staff is supported

i.To act as the Infection Prevention and Control Lead for the communityworking jointly with other managers and leads, to inform clinical practice.

j.To ensure effective multidisciplinary meetings for patient and familybenefit and safe, effective practice.

k.To liaise with external partners promoting relationships and partnershipworking, and to ensure timely and appropriate referrals.

l.To be proactive in developing our approach on inclusion and equityenabling access for all who need us.

m.Topromote the highest standards of holistic clinical and nursing care aredelivered and the use of compassionate, clear, and sensitive communication.

n.To ensure patient records are properly maintained in accordance withprofessional guidelines and Phyllis Tuckwell policy.

o.To be aware of current thinking, innovations and evidence base for communityservice care provision.

p.To ensure knowledge about patients is maintained in the strictestconfidence and is not divulged except to professionals, where it wouldmaterially assist with the management of care or protection of the individual.(See Staff Handbook Confidentiality and Data Protection Act).

q.To ensure that the standards set by the Care Quality Commission, NICEguidance, other national bodies and relevant safety notices are adhered to inaccordance with legal requirements.

r.To actin accordance with your professions Codes of Conduct.

2.0Leadership and Management

a.To be a visible leader and role-model for community colleagues atPhyllis Tuckwell.

b.To promote a culture of psychologically safe where colleagues wellbeingis promoted and supported.

c.To deputise for the Director of Patient Services in their absence asneeded (responsibility shared with other Managers).

d.To participate in the on call out of hours Manager/Lead rota

e.To investigate complaints, incidents or concerns from patients or theirrelatives, reporting outcomes of the investigation to the Director of PatientServices/Medical Director, Clinical Governance, and as appropriate, to externalorganisations e.g. Care Quality Commission and Safeguarding.

f.To effectively participate in and facilitate patient safety and qualityimprovement groups, to write and update relevant policies, procedures, andguidelines.

g.To help develop operational plans for an effective and efficientclinical service to patients and their families/carers.

h.To seek patient feedback and develop patient and family engagement in improvingour provision.

i.To contribute to the annual business planning process.

j.To be responsible for clinical budgets related to the line managed areas,working with the Director of Patient Services on preparations for budgetsetting.

k.To familiarise with Phyllis Tuckwell policies and guidelines particularlythose relating to health and safety at work, governance, medicines managementand risk management.

l.To represent Phyllis Tuckwell at relevant external meetings and engagewith healthcare colleagues as an ambassador and to promote whole system workingand ease of access to responsive end of life care.

m.To take responsibility for ensuring that appropriate clinical /managerial supervision is accessed by you and team members.

Experience

* Proven clinical and leadership experience at a senior clinical level and of managing teams.
* Experience of working within the speciality of Palliative Care/Oncology/ Frailty.
* Experience of a leadership role within the community and working in partnership with other organisations.
* Experience of whole system working
* A sound understanding of Hospices, end-of-life care priorities and initiatives both locally and nationally.
* Experience of change management
* Experience of Nursing workforce management and development.
* Demonstrable experience and knowledge of quality improvement, clinical audit, patient safety and clinical governance
* Experience of service development
* Experience of budget management


Qualifications

* Current registration with NMC
* Educated to degree level, or equivalent experience.
* Relevant continuing professional development in leadership and management; community practice, palliative and end of life care.
