What the Job Involves In this role, you will serve as a crucial contact point for young people leaving care, providing consistent support and coordination. You will work directly with them and their networks to ensure the achievement of personal plans and transitioning into adulthood. The job involves collaborating with various agencies, advocating for young people, and delivering targeted interventions and support for those at risk. Additionally, you will partake in a duty system to handle urgent support needs and ensure the completion of statutory Pathway Plans.
