Overview
The Hospital at Home (H@H) service based at Lorn and the Isles Hospital covers the Oban and the Lorn area. It is a developing service that has proven to be a safe alternative to hospital admission. The service provides acute care at home with the objective of reducing hospital admission and reducing length of stay in hospital. The Advanced Practitioner within the H@H service is a key role in the assessment and management of acute and complex patients; providing a high level of complex decision making within a multidisciplinary team. Management of patients with both acute and long term conditions across a number of different clinical pathways, where their medical conditions can be managed safely and effectively at home. We are looking for two posts in the Advanced Practitioner role to expand our team to provide a service across 7 days. The post will be fixed term or a secondment opportunity until March 2026.
