Job description

About the role:
To undertake assessments, care and support planning and reviews which are person centred and outcome focused. To work in an integrated and strengths-based way in community and/or hospital settings. to ensure health, wellbeing and social inclusion needs of citizens are met.
About you:
• Good communication skills both written and orally including the ability to communicate with individuals, carers, partners organisations and other professionals.
• Ability to organise, prioritise and manage work
• An aptitude for problem solving and an ability to negotiate and mediate in complex and challenging situations.
• An ability to collaborate with other professionals and partners when
working in complex situations.
• Good IT skills including ability to utilise Microsoft packages and data
bases
• Self-motivated, proactive, resourceful and able to recognise when to seek support
What you will do:
To dynamically assess the strengths and needs of individuals and/or their
primary carers using the established format and eligibility criteria within the Social Services and Well-being (Wales) Act 2014, Mental Capacity Act 2005 (MCA) and other relevant legislation for service provision. This includes complex and challenging work under supervision.
Embed positive risk taking within a strengths-based approach, to understand what matters to the individual and help them to manage risks to maximise choice and control over their lives. Where appropriate undertake Mental Capacity Act assessments and Best Interest decisions in line with the principle of the Act and the departments’ MCA Competency Framework This may involve managing the emotional and conflicting view of family/carers and other professionals where appropriate.
To co-produce person centred care and support plans for a range of settings. To meet identified outcomes which are flexible, strengths-based, and responsive to the changing needs of the individual. When required produce contingency plans.
To promote independence, self-determination, and choice through access to a range of services provided by the independent, voluntary and statutory sectors in line with the Social Services and Well-being (Wales) Act 2014 and other relevant legislation.
Prioritise assessments, reviews and manage workload in conjunction with the line manager. To provide support to other team members when necessary. To undertake duty on a rota basis. Recognise when to seek management or professional advice as appropriate.
Where appropriate, to work in a collaborative and multi-disciplinary way to achieve the best outcomes for individuals and their carers. To convene and chair care and support plan meeting as required. To attend meetings where appropriate, e.g. safeguarding, DSTs, Team Meetings.
To identify and report safeguarding concerns in line with safeguarding policies and procedures. To identify unmet needs and concerns about quality-of-service provision and pass this information to the relevant department.
