The Leeds and York Partnership NHS Trust has a national and international reputation in Addiction and Co-Occurring Mental Health Alcohol and Drugs (COMHAD) treatment. The COMHAD team, as part of Addictions Plus, LYPFT, works alongside other partners that make up Forward Leeds, a CQC outstanding rated organisation.
Following OHID (Office for Health Improvement and Disparities) investment and expansion of the service, an exciting opportunity has arisen for Band 6 Specialist COMHAD Practitioners to join our multi-disciplinary team. You will be joining a diverse and skilled team that consists of a Consultant Psychiatrist, Consultant Psychologist, Clinical Psychologists, Mental Health Nurses, Cognitive Behavioural Therapists, Social Workers, Occupational Therapists and Specialist Midwives.
Main duties of the job
The job role will include providing specialist addiction treatment, including psychosocial and pharmacological interventions, harm reduction, & risk management to service users with co-existing mental health and/or pregnancy needs. The LYPFT team is trauma-informed and offers a formulation driven, psychological model.
The successful candidate will hold a caseload as well as delivering psychosocial interventions to service users and consultancy to support the wider service. Excellent training from both Forward Leeds and LYPFT, supervision in a variety of forms and support from an experienced team will be offered.
There are excellent specialist training opportunities such as Dialectical Behaviour Therapy and a focus on career development. The successful candidate will also have opportunities to facilitate specialist psychosocial group work and an involvement with service development.

We also provide specialist inpatient services in York and some highly specialised services across the country. Detailed job description and main responsibilities
* Liaising with other teams within and external to Forward Leeds, other professionals, statutory and non-statutory bodies. To work effectively within the multi-disciplinary team.
* To develop and maintain excellent links with other agencies so as to ensure effective quality care for service users.
* Able to communicate with service users who may be distressed and who may, on occasion, present with challenging behavior.
* Advise healthcare professionals on the management of patients with co-existing physical health and substance misuse problems.
* To ensure safe practice and ensure that the service is effective and safe for service users, staff and visitors.
* To use clinical and line management supervision, as well as ad-hoc supervision if and when required, supporting decision making and managing complex situations.
* To report incidents in a timely manner using the Hub reporting system in line with Forward Leeds and Trust policies.
* Provide specialist clinical expertise with the provisions of assessments, care planning evaluation, risk assessment and follow up to patients.
* Manage a caseload and also deliver psychosocial interventions to service users within the wider service.
* Deliver a range of evidence led psychosocial interventions and support pharmacotherapy as appropriate with support from senior staff.
* Perform physical health checks and interventions such as venepuncture, immunisation, wound care, urine sampling.
* Participate in clinical supervision based on video recording of practice.

Qualifications
* Registered with professional body i.e. RMN/OT/Social worker
Experience
* Worked across life course, care settings, with ‘difficult to treat’ populations
* Experience of undertaking risk assessment and management of clients with complex needs and trauma
Skills
* Car driver with Full clean driving licence
