Rapid Elderly Assessment & Care Team (REACT)
Hospital at Home West Lothian – Hub St. Johns Hospital
12 months Fixed Term starting February 2026 – full time (40 hours per week)
Responsibilities
Provide acute assessment and treatment in the patient’s own home as an alternative to hospital admission. Development of skills in Medicine of the Elderly, frailty assessment, acute illness management, multi‑morbidity care and complex chronic disease management. Work closely within the multidisciplinary team under close Consultant supervision. One weekly session of development time for quality improvement, service development, teaching or subspecialty training.
Qualifications
Completed Foundation training minimum. Ability to drive a car for community home visits around West Lothian. Additional postgraduate experience and qualifications such as MRCP(UK) desirable. Good communication skills and the ability to work flexibly and collaboratively within a multidisciplinary team essential.
