Location: Kendal or Barrow in Furness. Contract Type: Permanent. Hours: Full time and part time hours will be considered; 2 x 37 hours posts available. There is an opportunity for one of the roles to work from the hospital setting at either Royal Lancaster Infirmary or Furness General Hospital.

We have an exciting opportunity to join our dedicated and supportive Hospital Social Work team. You will be part of a passionate, driven team focused on achieving positive outcomes for the people we support. The salary will be aligned to the career grade system based on competencies, experience, and qualifications, with progression through the grade as development occurs.
Responsibilities
* Carry out independent assessments and support planning within the Hospital Social Work Team.
* Balance risk, make sound decisions in unpredictable situations, and contribute to team planning and resource allocation.
* Participate in multidisciplinary discussions within the Transfer of Care Hub and support strength-based hospital discharge plans.
* Complete assessments and form support plans following discharge from hospital during a period of assessment.
* Engage in decisions on Discharge to Assess Pathways to achieve person-centred, community-based assessments and reduce delays to transfer of care.
* Provide duty cover within the Hospital Team to ensure continuity and quality transfer of care prior to assessment.
What we are looking for
A passionate and dedicated Social Worker with a Social Work Degree and registration with Social Work England. You will have experience in independent assessment and support planning, a good working knowledge of current legislation, theoretical models and best practice approaches, and the ability to create innovative, person-centred interventions. You should be confident in managing risk, making decisions in unpredictable situations, and collaborating with colleagues to plan and allocate resources. Strong communication, people management and collaboration skills are essential.
There is a requirement to work collaboratively within the Transfer of Care Hub for multi-disciplinary discussions and for Social Work intervention during hospital admission.
Team and roles
The primary role of the Hospital Team is to complete assessments and form support plans following discharge, during a period of assessment. There is an option to be based at Craven House/South Lakeland House or work from Furness General Hospital or Royal Lancaster Infirmary.
Applicants may be involved in decisions on Discharge to Assess Pathways to agree person-centred discharge plans and facilitate community-based assessments while reducing delayed transfers of care. A duty function within the Hospital Team may be required to ensure continuity and quality transfer of care prior to assessment.
