Hospital Social Worker - Neurosciences

An opportunity has arisen for a social worker to permanently join the specialist team based at Salford Royal Hospital, providing specialist social work support to patients who have suffered a stroke and have other long term Neurological Conditions. We invite experienced qualified social work practitioners (at least level 2 to level 3 post qualified) to apply for this permanent full-time post.
Main duties of the job
You will join an experienced and supportive Neurosciences Multi-Disciplinary Teams working across various wards and clinics and will gain valuable social work experience by working with this client group whose needs can be complex and challenging but very rewarding. You will provide social work assessments for the safe discharge of Salford residents, set up services as required, attend MDT and professional meetings while completing safeguarding investigations as required.
Ideally you will have some experience of working with this client group and of working in a hospital setting or similar.

Knowledge, Skills & Experience
* Understanding of the principles and values underlying the Care Act 2014, Mental Capacity Act 2005 and other relevant legislation
* Understanding of the integration agenda and the promotion of independence (enablement) model of health and social care
* Understanding of the social work role in a multi-disciplinary setting
* Experience of working with adults/older people
* Knowledge of relevant Welfare Rights Legislation
* Knowledge of the recent Government Guidelines
