Lincolnshire Community Health Service (LCHS) has an opportunity for Registered Nurses to join their Boston Community Nursing Team. We have vacancies for 1 full time position.
As a community staff nurse you will be required to work autonomously and as part of a team, delivering care to patients in their own homes, care homes and clinic settings. Successful candidates will be responsible for assessing care needs, and the planning, delivery and evaluation of nursing care using innovative evidence based practice.
Community nursing is exceptionally rewarding as it allows wonderful therapeutic relationships to be developed with patients in their own homes. Working in this capacity embodies the person centred care approach and allows patients to truly be at the centre of care delivery.
The skillset of a community nurse is vast and non‑exhaustive requiring knowledge and expertise in a range of areas. Examples of interventions provided are wound management, continence care, palliative care, medication administration, and long‑term condition management. Close working with the MDT (Social Workers, GP's and Allied Health Professionals) to ensure that patient care is effectively co‑ordinated, demonstrates a positive impact on our patients.
As this is a community post, you must be able to travel to patients' homes and clinic settings. Due to the rurality of Lincolnshire some areas are not well covered by public transport; therefore having access to transport is essential.
The service works through the hours of 8:00 am to 6:30 pm, 7 days a week, and is covered by variable shift patterns: Early shifts 8:00 am‑4:00 pm, 9:00 am‑5:00 pm, late shifts 10:30 am‑6:30 pm, and weekend shifts 8:00 am‑5:00 pm or 8:00 am‑6:30 pm.
The Community Nursing service delivers a 12‑week induction programme for all new staff. We also offer a robust preceptorship and clinical skills programme with an experienced clinical education team. This provides peer support in addition to the requisite training required to fulfil the role to its true potential.
To find out more about the key responsibilities and the specific skills and experience you will need, you are encouraged to look at the Job Description & Person Specification attachments under the Supporting Documents heading and refer back to these documents when completing your application.
