Job overview

The post holder is responsible for the delivery of general practice nursing services care to the practice population. Supported by experienced practice nurses within the Practice, they will deliver care within the boundaries of their role, focusing upon supporting patients to be healthy, monitoring of long-term conditions, health prevention and screening activities. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures and providing nurse leadership as required. They will be requested to do one late night.

Main duties of the job

The post holder will be an experienced nurse with key practice nursing skills and competencies and should also have experience in managing patients with a long-term condition. Supported by senior nurses within the practice, you will deliver care within the boundaries of your role, focusing upon supporting patients to be healthy, monitoring of long-term conditions, health prevention and screening activities. Experience of working in primary care essential.

Car owner/driver is essential.

This role will include working one late shift until 8.30pm (on a Tuesday) once every month.

We are looking to recruit a new practice nurse to join our friendly nursing team within a busy GP surgery in Leighton Buzzard. You will be expected to carry out tasks such as wound care, ECGs, blood pressure monitoring, cervical cytology, long term condition reviews and vaccinations as well as providing health promotion to our patients.

Detailed job description and main responsibilities

1. Assess, plan, develop, implement and evaluate programmes to promote health and well-being and prevent adverse effects on health and well-being
2. Implement and evaluate individual treatment plans for patients with a known long-term condition
3. Assess and care for patients presenting with uncomplicated wounds
4. Undergo relevant training to support and advise women requesting information relating to family planning needs
5. Undergo relevant training to support and manage the health needs of women presenting for cervical cytology consultations
6. Under the direction of experienced practice, nurses implement and participate in vaccination and immunisation programmes for both adults and children
7. Advice, support and where appropriate, administer vaccinations for patients travelling abroad

Education/ Qualification/ Training

Essential criteria

8. RGN with current NMC registration Evidence of Continued Professional Registration
9. Recognised qualifications in diabetes or chronic heart disease or respiratory medicine
10. A diploma in General Practice Nursing
11. Previous Experience in Primary Care
12. Qualifications/Certificates in Cytology, Travel vaccinations, Immunisations

Experience

13. General nursing experience
14. Working within Primary Care setting
15. Nursing management of chronic disease conditions in general practice, i.e. Diabetes, Asthma/COPD, Heart Disease

Knowledge and Skills

16. Excellent communication skills, both written and verbal
17. Change-management skills and ability to support patients to change lifestyle (Computer skills
18. Ability to apply clinical theory to practice
19. Evidence of Continued Practice Development
20. Awareness of current nursing issues and developments in General Practice Nursing
21. Aware of accountability of own role and other roles in a nurse led service
22. Awareness of clinical governance issues in primary care
23. Clinical governance issues in primary care
24. Identification of un-met health needs and programmes of health promotion and prevention of ill health
