This full-time locum role offers up to £27 per hour for an initial 3-month contract. What you will do: Conduct comprehensive occupational therapy assessments within community settings, including care homes and patients' own homes. Identify functional needs and establish personalised rehabilitation goals for patients on the Discharge to Assess pathway. Develop, implement, and review effective rehabilitation and care plans. Work collaboratively with multidisciplinary teams to ensure coordinated patient care. Monitor patient progress and adapt interventions to achieve the best possible outcomes. Provide advice, guidance, and support to patients and families to encourage engagement in rehabilitation programmes. Maintain accurate and up-to-date clinical documentation in line with professional standards. Requirements: HCPC registered Occupational Therapist. Previous community rehabilitation experience is desirable. Ability to work independently within community settings. Full UK driving licence and access to transport required for community visits.
