Overview
Our multidisciplinary teams work flexibly to provide a 7-day service, 365 days a year, between 8am and 8pm. Patients discharged from hospital into Home First receive step-down intermediate care, with community-based assessments and interventions in their own home to support recovery and monitoring in the early days post-discharge to prevent re-admission. Home First may provide acute reablement for up to 10 days to help people achieve functional goals and progress towards independence with activities of daily living. This wraparound recovery service from hospital discharge aims to maximise independence through enablement. We are looking for a Clinical Lead (Therapist) to join us. The post holder will provide Clinical and Professional Leadership, ensuring effective governance for staff working in Home First. They will collaborate with the AHP Head of Service and Professional Leads to ensure staff are clinically competent, services are clinically effective and safe, and an excellent patient experience is delivered. The post holder will embed the principles of interdisciplinary working and cross-skilling for all staff, including implementing competency frameworks. They will provide specialist clinical advice for complex patients and deliver specialist clinical assessment and intervention for highly complex cases as required. They will support an audit plan in the appropriate clinical area and participate in research as appropriate to inform future service development. The post holder will work as part of the team to develop key partnerships, ensuring effective whole-system working through communication and networking with other disciplines and by working across professional boundaries. They will ensure services continue to develop in line with Trust Strategy and act as deputy to the line manager.
You will need to be able to drive with a full UK licence and have access to a vehicle due to the geographic area covered by the team.
