Join Patient Navigation Services as our Team Leader
Lead a vibrant multidisciplinary team supporting adults and children with complex health and psychosocial needs. Our services (LaNS and PLaNS) help consumers and families navigate the health system, ensuring they get the right care, at the right time, in the right place.
As Team Leader, you'll provide clinical guidance, project support, and service leadership across both adult and paediatric pathways. You'll drive integrated, patient‑centred, trauma‑informed care and strengthen connections across primary, acute, and community services.

This is a multi-classified position with possibility of appointment at RN3.1, HP4 or P4. There is only one position available.
