Overview
Senior Clinical Pharmacist Home Treatment Teams based in Stockport and Oldham. You will join the Home Treatment Teams as an integral member of the multidisciplinary team and be responsible for delivering the clinical pharmacy service within your borough and providing Trust-wide HTT leadership. There is no dispensary or out-of-hours commitment, providing a practical opportunity to develop your clinical skills and broaden your mental health knowledge and experience.
Responsibilities
* Provide, manage, organise and develop the clinical pharmacy service to the HTTs across your borough and lead developments to the clinical pharmacy service to HTTs Trustwide.
* Lead, deliver, develop and evaluate specialist clinical mental health pharmacy services in the Home Treatment Teams.
* Provide expert pharmaceutical advice and liaison to the Home Treatment Team, GPs, practice pharmacists, other healthcare professionals, patients and carers.
* Act as an independent prescriber of medication where appropriate, ensuring drug therapy is evidence based, safe, cost effective and appropriate to the individual patient.
* Provide training to the Home Treatment Team, GPs and other HCPs within Primary Care around the use of psychotropic medicines.
* Ensure that pharmacy services work within agreed guidelines to ensure effective medicines management.
* Provide cover for other pharmacists providing a service to and employed by Pennine Care NHS Foundation Trust, as requested.
* Work within a multi-disciplinary Home Treatment Team in Stockport and Oldham and act as clinical pharmacy service lead for HTTs across the Trust.
* Be supported by a clinical pharmacy team dedicated to delivering services across community, assessment, specialist and inpatient services.
* Receive clinical and management supervision and have opportunities to develop yourself and the role through education, training, team meetings and opportunities such as Quality Improvement and audits.
* Be supported to undertake further qualifications as needed.
* The role is entirely clinical with no dispensary, out-of-hours or on-call commitment.
