We are seeking an experienced and compassionate Advanced NursePractitioner (ANP) to join our Enhanced Primary Care Mental Health Service. As a vital member of ourintegrated multidisciplinary team, you will help provide holistic, coordinated,and person-centred care that improves outcomes and enhances quality of life. Inthis role, you will deliver personalised support for individuals living withlong-term health conditions (LTCs) who are also managing a mental healthcondition.

You will play a key role in improving health outcomes, reducing healthinequalities, and empowering patients to manage their conditions through shareddecision-making, physical health monitoring, and tailored health interventions.You willcontribute to a multidisciplinary team by fostering a culture of reciprocallearning. This will involve equipping colleagues from mental health backgroundswith new skills while also drawing on their expertise. Through this two-wayknowledge transfer, the team will build shared understanding, strengthencollective capability, and drive more integrated and effective ways of working.


Main duties of the job

TheANP will work autonomously and as part of a team to support earlyidentification of physical health issues, provide ongoing management ofco-existing physical and mental health conditions, and ensure equitable accessto healthcare for this vulnerable population. The post holder must be aqualified independent prescriber and will utilise their clinical skills todiagnose, manage, and prescribe as appropriate. The ANP will be clinicallyresponsible to prescribe in the absence of a GP.

Job responsibilities

ClinicalPractice:

Conductphysical health assessments and screenings for patients with long-term mentalhealth conditions, including cardiovascular risk assessments, diabetes checks,respiratory assessments, and metabolic monitoring.

Undertakeannual physical health checks for patients receiving shared care (e.g.,antipsychotic monitoring in partnership on behalf of primary care).

Interpretclinical findings and initiate appropriate treatment plans or referrals.

Prescribemedications safely and appropriately, in line with local and nationalguidelines.

Manageand review physical health issues in collaboration with mental health andprimary care teams.

Promotelifestyle advice and health education (e.g., smoking cessation, diet, physicalactivity).

Maintainaccurate and up-to-date clinical documentation in line with local and nationalpolicies.

Leadershipand Service Development:

Contributeto service planning and quality improvement initiatives focused on integratingphysical and mental healthcare.

Supportjunior staff and contribute to the clinical supervision and training ofcolleagues where appropriate.

Liaiseeffectively with GPs, community teams, pharmacy, and other healthcareprofessionals to ensure continuity of care.

Governanceand Professional Practice:

Adhereto clinical governance frameworks including risk management, incidentreporting, and audit.

Maintainown professional development in line with NMC/HCPC registration andrevalidation requirements.

Followall local and national policies relevant to the ACP role, prescribing, andmental health services.


Person Specification


Experience

* Significant post-registration experience in mental health or physical health settings.
* Evidence of delivering holistic, patient-centred care.
* Experience of audit, service evaluation, or quality improvement projects


Knowledge and Skills

* Competence in managing long-term conditions and conducting physical health assessments
* Strong communication, clinical decision-making, and risk management skills
* Experience of using EMIS Web Clinical System
* Shared Care Prescribing


Qualifications

* Registered Nurse with valid NMC registration.
* MSc in Advanced Clinical Practice or equivalent
