Job Description
Occupational Therapist – Adult Social Care. Working Pattern: Community-based with flexible working options. Local Authority located in Herefordshire is seeking a qualified, HCPC-registered Occupational Therapist to join its Independent Living Service. This role focuses on supporting adults of all ages to maintain independence, wellbeing, and quality of life through strengths-based, person-centred practice within the community.
Main Duties
* Complete statutory Occupational Therapy functional, social, and environmental assessments
* Apply a strengths-based approach to identify risks, barriers, and preventative strategies
* Support adults in their own homes to engage in meaningful daily activities
* Recommend and facilitate provision of equipment, minor adaptations, and specialist community resources
* Undertake moving and handling assessments promoting dignity, safety, and independence
* Work collaboratively with social workers, health professionals, housing teams, families, and carers
* Maintain accurate documentation and work in line with safeguarding and local authority procedures
Requirements
* Degree or Diploma in Occupational Therapy
* HCPC Registration
* Experience working in Adult Social Care or community settings
* Knowledge of relevant legislation and person-centred practice
* Strong communication, organisational, and caseload management skills
* Eligibility to work in the UK
* Enhanced DBS (assistance available)
