The Cancer Division at University Hospitals Sussex NHS Trust (UHSx) is looking to recruit to a0.8 WTE co-ordinator for a newly funded project for onco-frailty patients. The purpose of this is to create and deliver a stratified approach to frailty with a multifunctional generic toolkit at the beginning of the pathway & access to specialist review and services at the end.

This role will offer the postholder an opportunity to be part of an innovative project, develop leadership & project management skills, deliver tangible recommendations and work across a variety of clinical specialisms, ie, oncology, care of the elderly and palliative care.

Main duties of the job

Communication

1. To communicate with internal and external stakeholders and patients/clients so that they are involved in the delivery of the service and pathway transformation and improvements.

2. Have contact with patients and their relatives, who may be distressed and anxious, and to usegoodcommunication skills to provide supportive listening

3. To communicate effectively with the project team

4. To ensure that written and electronic records and activity data are maintainedin accordance with trust standards

5. To support the bringing together of diverse stakeholder groups with differing agendas to achieve mutual agreeable outcomes / deliverables

6. Work successfully with complex stakeholder groups, often dealing with ambiguity, complexity and conflicting opinions / views

7. To support the project lead and team members in the negotiation with all relevant stakeholders, in the scope, size and timings of projects and any relevant project management and support required

8. Provide a consistent and knowledgeable contact point, answering queries, giving advice and providing information about the project to relevant stakeholders

Service Delivery & Improvement

9. To work effectively as a member of a team to support the planning and achievement of onco-frailty project objectives
10. To manage the project from an administrative perspective supporting clinicians to ensure all information is collated, in a timely manner and can be used to make decisions on how project can progress.
11. The postholder will be responsible for making sure the project infrastructure and data outputs are in place to make the project a success and that there is appropriate governance.
12. To operationally manage the MDT meetings and patient clinics, including creating patient lists, booking patients' appointments, sending letters, phoning patients to check attendance, sending discharge letters
13. Be responsible for the generation of all requisitions relating to the project including leading on budget planning and project reconciliation at a granular level.
14. To prioritise spending requests and authorise appropriate payments, through the relevant organisational processes.
15. To contribute to the management of third parties (such as consultants/interims) to ensure deliverables are met in a timely manner and within budget.
16. To support the development and delivery of audit activities to measure service change and sustainability involving complex data collection and analysis
17. To ensure date flow to the business intelligence team to collect service outcomes

People Management & Development

18. Responsible for carrying out duties in line with the Trust’s e-rostering processes

19. To be available to all members of the multi-disciplinary team to provide advice on all administrative matters

20. Together with the Project lead, to produce clear output documentation of work and store it appropriately on the shared area, this will be required regularly and ad hoc.

Patient Care Delivery

21. Work effectively with diverse patient groups to involve and engage users and carers in the delivery of service and pathway transformation and improvements
22. To guide patients through the service, including phone calls to explain the service and to book appointments
23. To contact patients and support with the collection of patient reported outcome measures. This might be asking questionnaires over the phone or guiding a patient through a digital system
24. To be present in clinics to guide patients through the clinic from waiting room to their correct consultations
25. To work with patients, face to face or over the phone to collect outcome measures
26. To guide patients in using digital systems such as patient knows best to complete outcome measures
27. To coordinate MDT lists, prepopulating meeting proformas with relevant clinical information from other systems
28. Provide information and support for patients with onco-frailty needs.

Learning & Development

29. Attend mandatory training updates as required.

30. Undertake training as necessary in line with the development of the post and as agreed with the line manager as part of the personal development process.

31. Identify own learning needs and jointly plan training requirements with your line manager

32. Participate in the Trust’s appraisal process to discuss how your role will help deliver the best possible care to our patients and help to deliver any changes in service.

33. To provide support and training to other similar posts within the department
