Job summary
* This is an exciting new opportunity for a Advanced Clinical Pharmacist (ACP) to develop their career in a primary care setting.
Main duties of the job
To further develop and enhance the clinical pharmacy service to the patients of the practice. Support the practice to achieve maximum performance through QOF including ensuring good working relationships with Community Pharmacy and supporting Practices to achieve targets for prescribing practice.
o Long-term condition reviews - Take responsibility for the care management of patients with chronic diseases where medicine optimisation is required. E.g., COPD, asthma.
o Patient facing clinical medication reviews - Undertake clinical medication reviews with patients and make appropriate recommendations for the clinical team on prescribing and monitoring. Through structured medical reviews the post holder will support patients to take their medications to get the best from them, reduce waste and promote self-care.
o Opportunity to develop specialist areas of prescribing expertise in line with the practice's clinical priorities
Provide specialist expertise in the use of medicines whilst addressing both the public health and social care needs of patients to proactively tackle inequalities in health resource usage and outcomes in the area.

· Demonstrate understanding of the pharmacy role in governance and is able to implement this appropriately within the workplace

· Specialised knowledge of drugs used and therapeutic trends within specialised area
· Excellent oral and written communication skills

Qualifications
Essential
Pharmacy Honours degree (BPharm or MPharm)
Postgraduate Clinical Pharmacy Diploma
Registration with the General Pharmaceutical Council (GPhC)
Non-medical prescriber (Pharmacist Independent Prescriber) or willingness to complete the NMP course to attain the necessary qualification

Substantial post registration experience in Primary Care
Experience of multidisciplinary team working and making clinically appropriate interventions
Demonstrable experience of cross sector collaborative working with primary care healthcare professionals
Experience of writing and reviewing policies, guidelines and SOPs
Experience of service development
Experience of line management and clinical supervision
Experience and competence to provide patient information & education to patients, families and carers
Experience of undertaking clinical audits and improvement projects leading to service improvements
Ability to demonstrate an understanding of the key stakeholders the post is likely to be supporting and how they will be expected to support them.
