Join the Community Therapy Team – North (Mid‑Sussex, Horsham & Crawley). We support people in our communities to live as independently as possible in ways that truly matter to them. We’re offering opportunities for Band 4 Therapy Practitioners to join our friendly teams in Mid‑Sussex and Horsham.
Responsibilities
* Deliver tailored rehabilitation programmes to help people regain independence.
* Support those with long‑term conditions to return to baseline function or adapt to new abilities.
* Provide reablement so people can live more independently at home and in the community.
* Offer equipment, advice, and practical solutions to support self‑management.
* Empower patients to set and achieve meaningful goals.
* Work closely with patients in their own homes to assess and improve skills in activities of daily living, mobility and self‑care.
* Complete administrative tasks, order equipment, liaise with family members, carers and professionals, and maintain good electronic records.
* Travel independently across the geographic area covered by the team and work within operational hours.
* Work under the direction and supervision of qualified Occupational Therapists, Physiotherapists and other therapy assistants.
Qualifications
* Relevant therapy experience.
* Demonstrated willingness to complete specific therapy competencies and further learning as required.
* Full UK driving licence and access to personal car.
* Strong communication, interpersonal and organisational skills.
* Willingness to learn and develop.
