Southampton West OPMH CMHT is seeking committed and qualified professionals to join their team based at Western Community Hospital, Millbrook. The Band 6 position offers a thrilling opportunity to conduct mental health assessments and provide advice and support to our clients.
Prior experience with the elderly and individuals with severe mental illnesses is preferred, whether in a hospital or community setting. Expertise in Dementia care and assessment would also be an asset for this position.
The role of a Community Care Officer (CCO) is both challenging and fulfilling, serving as the initial point of contact for the elderly facing mental health challenges and significantly influencing the care we deliver in our community.

Main duties of the job
The Senior Community Mental Health Practitioner will work as a core member of the Community Mental Health team to coordinate the care of a complex caseload of service users, supervising a Band 5 Community Practitioner or Band 3 Community Healthcare Support Worker where appropriate. Working under the supervision of the Deputy or Team Manager, the practitioner will formulate care and risk management plans based on identified needs, ensuring these are recovery-focused and developed collaboratively with the service user and, where appropriate, their relatives and carers.

Salary: £38,682 to £46,580 per year, based on full-time hours
