Overview
The Uro-Oncology CNS plays a pivotal role in delivering high-quality, evidence-based, and patient-centered care to individuals diagnosed with Urological cancers. Serving primarily as the patient's cancer keyworker, the post-holder provides expert clinical leadership, education, advocacy, and support to patients and their care partners as well as the multidisciplinary healthcare team, through all stages of the care trajectory from initial referral and diagnostics, through treatment, follow up, and beyond.
Responsibilities
* Undertake advanced patient assessment, advising on immediate care or referral for medical interventions, ensuring the prompt and effective management of patients.
* Serve as the Cancer Keyworker, empowering Urological Cancer patients and their partners with person-centred information and support, and facilitate communication and decision making between them and the multidisciplinary team.
* Effectively manage an ongoing caseload of patients, working within the multidisciplinary team to facilitate care and refer patients to specialist professionals, as necessary.
