We are looking for committed Healthcare Professionals r to join GPDQ in collaboration with Berkshire West GP Federation working within an Urgent Care Centre. In this role, you will triage patients in the emergency department, using your clinical judgement to assess the most appropriate treatment centre, ie ED or the Urgent Care Centre.
Responsibilities
Patient Assessment & Care:
Dive into each patient’s case with a comprehensive assessment, leveraging your advanced clinical skills to swiftly pinpoint the best course of action.
Tackle patient needs with a sharp focus on clinical urgency to ensure timely and effective care.
Coordination & Referral:
Team up with GPs, paramedics, and other healthcare professionals to create a smooth and coordinated care experience, ensuring patients transition seamlessly between services.
Connect patients with the right healthcare providers, specialists, or emergency services based on your thorough clinical evaluations.
Patient Education & Support:
Empower patients and their families with clear, actionable information on managing conditions at home.
Guide them through lifestyle adjustments, medication routines, and recognize signs that require further medical attention.
Documentation & Record-Keeping:
Keep meticulous, up-to-date records of patient assessments, treatments, and care plans, following all regulatory and organizational standards.
Ensure every patient interaction is accurately logged in the appropriate system to maintain comprehensive records.
Continued Professional Development:
Engage in ongoing training and professional development to stay on the cutting edge of medical guidelines, protocols, and best practices.
Qualifications and Experience required
Must be a registered Nurse or Paramedic
Hold an independent prescribing qualification (not essential)
A minimum of 6 months experience working within a Primary Care Setting
Strong clinical assessment skills
Ability to work autonomously and make informed decisions under pressure
Proficient in Emis
