Overview
To provide and maintain high quality effective continence re-assessments to housebound patients or clinic patients who require continence management products, working under supervision of the continence nurses within the Continence Advisory Service. To deliver high quality, evidence-based care for patients with or at risk of compromised continence in their own home or within a clinic setting. To support other services to deliver continence support as part of a holistic approach to the health and wellbeing of the people we care for.
For staff already employed by the Trust as a band 3 Clinical Support Worker, you should instead apply through the Clinical Support Worker transfer process via your line manager.
Responsibilities
* Under the direction of the continence nurses undertake continence product reassessments for housebound and clinic patients reflecting the needs of individuals.
* Have a good working knowledge of continence products that are available to help maintain a patient’s continence requirement.
* In line with training and achieved competence, carry out clinical procedures as directed and required (for example pre and post void bladder scan).
* Achieve and maintain clinical competences relevant to your role and area, undertaking training as appropriate and agreed by the operational lead to develop performance and support safe practice.
* Acknowledge limitations of own competence and not undertake work outside the scope of your role.
* Ensure effective communications with patients, carers and all other health and social care professionals regarding all aspects of patient care, ensuring issues are reported promptly.
* Maintain confidentiality and information security in accordance with department, Trust and national policies/guidelines, reporting any breaches to senior colleagues.
* Be responsible for the quality of data you record, ensuring data is accurate, kept up to date and stored appropriately.
* Develop and maintain effective appropriate communications systems.
* Work in partnership with other members of Sheffield Teaching Hospitals NHS Trust and with statutory and voluntary agencies.
Qualifications and requirements
* Good working knowledge of continence products and how they support patient needs.
* Willingness to undertake training and develop clinical competences relevant to the role.
* Be able to work under supervision and recognise limitations of competence.
* Ability to communicate effectively with patients, carers and health and social care colleagues.
* Maintain confidentiality and information security in line with policy.
* Must be a car driver with a valid driving licence due to the requirement to undertake city-wide visits.
