Job Summary

Our PCN is looking for a physiotherapist for between 15 and 30 hours per week to join the PCN's wider MDT team. The PCN has its own building in South Shoebury and covers four practices. The role involves working autonomously in your own clinics with clinical support from other clinicians in the PCN. Shift times are flexible, including options for later evenings and Saturdays with enhanced pay rates.

Main duties of the job

Applicants should be fully HCPC and CSP registered physiotherapists with at least 5 years post-graduate experience, including 3 years in MSK clinical practice as per HEE's Roadmap to Practice. The role requires a driven personality passionate about developing clinical skills, flexibility to adapt to the fast-paced primary care environment, and the ability to work to 20-minute appointment slots. Responsibilities include identifying patients needing joint injections, referring to other clinicians, and working as part of a wider MDT to deliver PCN projects such as the CVD LES.

About Us

Southend East PCN, located at Norton Place in Shoebury, serves approximately 37,000 patients across five practices. The team includes pharmacists, pharmacy technicians, social prescribers, health trainers, paramedics, physiotherapists, and assistant practitioners, supported by clinical directors and an operations manager. We have a strong digital presence and employ a Digital Transformational Lead to support our network.

Job Responsibilities

Clinical Practice

* Assess, diagnose, triage, and manage patients with complex caseloads, progressing investigations and referrals as trained (e.g., x-rays, blood tests).
* Utilize full clinical scope, including independent prescribing, injection therapy, and investigations when qualified.
* Manage complex psychosocial and mental health interactions, referring to social prescribing as needed.
* Deliver supported self-management programs focusing on behavioral change, physical activity, mobility, and independence.
* Provide tailored care, considering the impact of clinical status on overall health and employment.
* Support patients in setting goals and self-management confidence.

Evidence, Research, and Development

* Develop collaborative relationships across the PCN to support pathway integration.
* Work as part of an MDT using MSK expertise to strengthen links to wider MSK services through leadership, teaching, and evaluation.
* Support research and audit to evaluate and improve the FCP service.
* Streamline care pathways for timely patient access.

Leadership

* Lead and contribute to service delivery and clinical governance, including regular audits, supervision, and training.
* Communicate effectively with patients and carers about diagnosis, pathology, prognosis, and treatment options to support personalized care.

Facilitating learning

* Provide feedback and support to PCN clinical team members on referral criteria.
* Reflect critically on practice and pursue ongoing development.
* Collaborate across the multidisciplinary team to create and evaluate clinical pathways and services.
* Lead MSK clinical and service development, providing learning opportunities within primary care.
* Build collaborative relationships across the PCN to support pathway integration and service development.
* Engage with secondary and community care services, utilizing local interventions to support patient management.
* Support regional and national research and audit programs, sharing outcomes to enhance practice and pathways.

Person Specification

Qualifications

* Essential: Fully HCPC and CSP registered physiotherapist.
* Desirable: Level 7 MSc qualification, qualified injection therapist and/or independent prescriber.

Experience

* Essential: Minimum 5 years post-grad physiotherapy, with at least 3 years MSK clinical experience.
* Desirable: Experience in first contact triage, GP practice work, and using EMIS/SystmOne.

ARRS Requirements

* Completed undergraduate physiotherapy degree.
* Registered with HCPC.
* Holds relevant public liability insurance.
* Masters level qualification or equivalent expertise.
* Demonstrates Level 7 MSK practice capability or equivalent.

Other

* This post is subject to DBS check under the Rehabilitation of Offenders Act 1975.
* Current UK professional registration required.
