Applications are welcome for a fixed term contract for 9 months to work as a Post Diagnostic Support Worker for people living with Dementia.
An exciting opportunity has become available for an experienced practitioner with relevant qualifications / experience to work as part of the Older Adult Team in Lochaber working alongside the National Alzheimers Scotland Team.
The Older Adult Team work as part of the Lochaber Community Mental Health Team who support adults over 65 years of age with Mental Health diagnosis such as Dementia.
Applicants should have experience working with Older Adults or Mental Health. They should hold minimum of SVQ Level 4 in Health and Social Care, SQA Level 9 or a current professional qualification in Health Care.
Full UK Driving Licence and availability of use of own car for work purposes.
The team in Lochaber work with a Multidisciplinary approach and as such applicants must have excellent communication, good IT skills and team working skills.
This is a case load holding position for supporting adults living with Dementia in the first 12 months post diagnosis. The successful applicant will support planning for the future to develop an outcome focussed care plan for clients and their families.
Please be advised this role does not meet the eligibility criteria for a Skilled Worker Visa or a Heath and Care Worker Visa.
