An opportunity has arisen for a month fixed term contract within the heart failure team (ending th March ) at the Oxford University Hospitals NHS Trust. Covering the John Radcliffe from Monday to Friday.
This is a great opportunity to work where your existing knowledge and expertise in cardiology/heart failure will be fully utilised within a service that has regular team meetings, clinical support and a strong culture of learning together.
The successful applicants will join the existing Heart Failure team in order to:
•Assess all patients admitted with heart failure
•Provide information and advice to patients and families/ carers whilst they are in hospital
•Liaise with primary care based clinicians, particularly the community based Heart Failure nurses in Oxfordshire and surrounding counties, in order to maximise continuity and quality of care
•Undertake the necessary data entry and audit requirements of the Trust in order to monitor treatments and quality of care
The successful applicant will be a registered nurse with experience in cardiac nursing, ideally experience working with the chronic heart failure population. They will provide evidence of post-registration study and demonstrate post-graduate level working via a continuous development portfolio.

Main duties of the job
The successful applicant will be a registered nurse with evidence of relevant continuing professional development with excellent verbal and written communication skills. They will demonstrate a keen interest in the field of heart failure nursing and show evidence of experience of working with people across the spectrum of chronic heart failure, from diagnosis to end of life care, and including acute decompensations. They will promote and ensure the delivery of, high quality, multidisciplinary, evidence-based care throughout the patient pathway for patients and their families.
Contribute to the professional development of the heart failure service at a local level, providing nursing expertise for patients, families and carers.
Assess patients admitted with confirmed or suspected heart failure, and collaborate with the MDT to advise on investigations and management plan. The post holder will provide information, education, and support to patients and family/carers, and will contribute to audit processes.
Act as a resource for nursing colleagues, contributing toward and providing educational activities in order to ensure the development of effective, evidence based, multidisciplinary practice.
