Overview
The Health Care Assistant will provide person-centred domiciliary care to adults within their own homes in Askam-in-Furness (LA16 7BN) and surrounding localities. Care is delivered in accordance with:
* Individual Care Plans
* Risk Assessments and Moving & Handling plans
* Medication Administration Records (MAR)
* Local Authority commissioned care packages
* Provider governance and compliance policies
The role promotes independence, dignity, safety and wellbeing through a strengths-based and outcome-focused approach.
Responsibilities
A. Personal Care Delivery
* Assistance with washing, bathing, dressing and grooming
* Toileting and continence support
* Safe moving and handling in line with risk assessments
* Medication prompting or administration (where trained and authorised)
* Nutritional support, hydration monitoring and meal preparation
* Light domestic tasks as specified within commissioned packages
B. Complex and Specialist Support (Where Commissioned)
* Dementia care
* Physical disability and mobility support
* Epilepsy awareness and monitoring (where relevant to care plans)
* End-of-life / palliative care
* Learning disability support
* Positive behaviour support strategies
C. Safeguarding and Risk Management
* Immediate reporting of safeguarding concerns
* Adherence to Cumbria safeguarding procedures
* Infection prevention and control compliance
* Accurate and timely documentation of care provided
* Completion of daily care notes and electronic or paper records
* GDPR and confidentiality compliance
Working Arrangements
This role involves lone working within service users' homes across Askam-in-Furness (LA16 7BN) and nearby communities within the Furness area of Westmorland & Furness Council. Travel between service users may be required depending on care package allocation and rota scheduling. A full UK driving licence and access to a vehicle is desirable due to the geographical spread of domiciliary care visits.
