What you will do:
* Work with young people transitioning from children’s to adult services, assessing their needs and developing comprehensive support plans.
* Collaborate with families, carers, and other professionals to ensure a holistic approach is taken in providing the transition of care.
* Advocate for young people, ensuring their voices are heard and empowerment is at the forefront of all decisions.
* Monitor and review support plans, adapting and updating them as necessary to ensure the best outcomes for young people.
* Maintain accurate records and ensure compliance with all relevant policies and procedures.
