Responsibilities
Meridell Achievement Center is a nationally recognized 134-bed private residential treatment facility that specializes in the treatment of children and adolescents with emotional, behavioral and neurobehavioral problems. The center provides a range of services and programs with evidence-based treatment and a mission to provide safe, effective, and compassionate mental health and substance use treatment. Meridell is a subsidiary of Universal Health Services, Inc. (UHS).
Meridell is looking for an experienced Case Manager to work with a supportive team and our male adolescent population. The case manager collaborates with therapists to coordinate services for residents, assists with transitions from the facility, and ensures timely intake, discharge planning, and aftercare planning.
* Ensure intake is completed for new admissions, including psychosocial assessments, initial treatment plans, and outcome measures.
* Initiate discharge planning at admission and coordinate with therapists to plan aftercare services prior to discharge.
* Collaborate with Utilization Review staff and program therapists to maintain authorization status and anticipate discharge needs.
* Complete master treatment plans for new admissions.
* Secure aftercare provider appointments within required timeframes and facilitate admissions screenings for aftercare providers (e.g., partial hospital programs, residential treatment centers, group homes).

Qualifications
* Education: Must have a Master's degree from an accredited college or university in a mental health field and a current State of Texas license as a PhD, PsyD, LCSW, LMFT, LPC, LMSW, LMFT-A, or LPC-A, obtained prior to hire date or job transfer date.
* Experience: Four years of work experience in mental health or related field preferred. Two years direct practice experience and/or clinical training specific to individual, family and group therapy (if a specialized issues group, training must be specific to the issue).
* Skills: Strong verbal and written communication skills; ability to communicate clearly with children and adolescents, families, interdisciplinary team members, and managed care representatives. Clinical skills to assess dynamics and develop effective treatment strategies within the overall treatment plan. Strong analytical ability to evaluate information and revise treatment plans. Leadership abilities to support clinicians, nurses, and care workers. Flexibility regarding program needs.
* Essential Physical Requirements: Ability to perform medium physical work including walking, sitting, or standing.
