Job Overview:
This position provides both direct and indirect patient care in a primary care office. The role involves working with care delivery providers to identify care gaps, contacting patients for scheduling, and providing referral follow-up. The Medical Home LPN handles pre-visit planning, manages messages through electronic portals, and assists in transitions of care. Acting as a clinical liaison, the LPN communicates actively with patients, participates in process improvements, and supports clinical goals including patient satisfaction and engagement. Other duties may be assigned as needed. The ideal candidate will have strong skills in clinical care, customer service, communication, and teamwork, supporting the mission and values of TriHealth Physician Practices.
Job Requirements:
* Graduate of an approved healthcare technical, professional, or vocational program (or equivalent experience)
* Preferred experience in a physician practice or related healthcare field
* Basic Life Support (BLS) and CPR certification
* Knowledge of medical office flow, clerical/front office tasks, EMR, practice management software, and medical coding/billing (strongly encouraged)
* 3-4 years of clinical healthcare experience
Job Responsibilities:
* Coordinate primary care rooming, medical procedures, and patient care including immunizations, venipuncture, point-of-care testing, and retinal scans. Follow protocols for clinical procedures and equipment use.
* Document all aspects of care accurately, including clinical calls, procedures, prescriptions, and workflows. Manage messages timely, utilizing MyChart messaging for patient communication.
* Participate in the patient-centered medical home team during visits, review patient charts for care gaps, and assist with outreach campaigns to close these gaps.
* Support pre-visit planning, participate in daily huddles, and promote wellness and preventive care by reminding patients of screenings and immunizations.
* Understand population health concepts, value-based contracts, and quality metrics. Identify at-risk patients and participate in related training.
* Assist with post-discharge outreach, collaborate with care team members, and provide community resources and patient education for wellness and chronic disease management.
Other Job-Related Information:
Experience with multiple age groups, understanding of screenings and chronic disease management, and familiarity with patient-centered medical home principles are preferred.
