An exciting opportunity has arisen for an individual to be part of a forward‑thinking team supporting multidisciplinary teams and patients with discharge planning on the Older Persons Admissions Unit (OPAU).
Responsibilities
* Support multidisciplinary teams and patients with discharge planning on the OPAU.
* Assess and develop individualised, appropriate and timely discharge plans for patients.
* Collaborate with medical, nursing and therapy teams to assess discharge needs early in the admissions process.
* Liaise with community services, social care and families to ensure continuity of care, providing clear communication between hospital and external agencies.
* Monitor and reduce delayed discharges and support patient flow across the unit.
* Use competent I.T skills to facilitate efficient discharge processes.
* Problem‑solve complex situations and recommend appropriate action, often under pressure.
* Adapt to flexible work patterns to meet the needs of the service.
Qualifications
* Some experience working within a healthcare setting.
* Good understanding of community services.
* Competent I.T skills.
* Strong communication skills across face‑to‑face, telephone, email and MDT working.
* Pro‑active approach towards discharge planning and ability to prioritise workload.
* Willingness and commitment to learn, with a passion for delivering high‑quality patient care.
Additional Information
Base Location: Royal Bournemouth Hospital.

If a role or service relocates as part of a planned move, excess mileage will not be reimbursed. Travel from home to the new work base will be classed as a normal commute. Any other changes will be managed under Trust or national terms and conditions.
