Job responsibilities
The post is to work within and as part of the Community Health Care team supporting both the clinical and administrative teams to promote and facilitate effective leadership for community healthcare and to support the discharge coordinator in providing safe and effective care and rehabilitation for patients in their own homes.
* Support day‑to‑day leadership and management of the community health care team under the direct management of the discharge coordinator.
* Supervise and coordinate Short Term Care staff, ensuring patient care is carried out according to the care plan and as delegated by the registered nurse.
* Assist in the induction of new staff, conduct supervision of junior staff members, assess competencies, complete rotas and manage referrals into and out of the service using specific criteria.
* Carry out delegated work in accordance with the competencies detailed within the NQF at Level 5 foundation degree level.
* Provide direct patient care under indirect supervision of a Registered Nurse, completing individual patient care plans and reporting any changes in the patient’s condition in a timely manner.
* Offer support to ensure patients receive a professional and caring service within the necessary clinical timescales.
Person Specification
Education and Qualifications
* Evidence of work‑based competencies associated with the role.
* GCSE or equivalent English and Maths Grades A‑C.
* NVQ level 3 in a healthcare related area.
* Computer literacy to a level consistent with everyday use of software programmes, System 1 email and internet.
* Certificate in Higher Education in Effective Practice in Care or equivalent.
Knowledge, Training & Experience
* Previous experience of working within a health care setting for a minimum of 3 years.
* Knowledge of how to deal with non‑routine activity such as problem solving and work allocation.
* Proven ability to work on own initiative.
* Supervisory experience.
* Experience of dealing with difficult situations.
* Clear understanding of the needs of patients who require enablement services.
* Awareness of professional boundaries and responsibilities.
* Excellent communication and interpersonal skills.
* High level of initiative and proven organisational skills.
* Ability to manage a heavy workload without supervision.

* Driving licence.
