Adults Social Worker – Disabilities Specialist

Job Details
* Employment Type: Temporary (26 weeks, fixed-term project)
* Number of Positions: 2
* Start Date: 01/10/2025
* Hours: 37 hours per week
* Pay Rate: PAYE: £26.55 per hour | Umbrella: £34.03 per hour
* Location: Newtown
Key Responsibilities
* Conduct comprehensive assessments of adults with disabilities, identifying strengths, support networks, and care needs to promote independence and well-being.
* Develop and implement personalised care and support plans, collaborating with individuals, families, carers, and multi-disciplinary teams including health board partners.
* Safeguard vulnerable adults, manage risks, and ensure compliance with the Social Services and Well-being (Wales) Act 2014.
* Facilitate access to community resources, day services, and preventive support to reduce reliance on statutory interventions.
* Maintain accurate records, contribute to reports, and participate in reviews to monitor outcomes and adjust plans as needed.
* Promote person-centred practice, advocating for service users and liaising with third-sector organisations for holistic support.

Requirements
* Essential: Qualified social worker with registration on Social Care Wales.
* Essential: Proven experience in adult social care, particularly with disabilities, including assessments, care planning, and safeguarding.
* Essential: Strong communication skills, empathy, and ability to work collaboratively in a multi-agency setting.
* Desirable: Knowledge of Welsh Government policies and rural service delivery challenges.
* Must have: Eligibility to work in the UK, DBS Summary Form, Social Care Wales Registration, and 5 years of references/work history; Confidentiality Agreement required upon offer acceptance.
