An exciting opportunity to join the all age continuing care service covering the North East and North Cumbria as a children's continuing care (CCC) nurse assessor/case officer, potentially covering the North Cumbria locality. The service undertakes assessments and commissions packages of care across the North East and North Cumbria in line with the National Frameworks for Continuing Healthcare and Children's Continuing Care. Responsibilities
* Clinical assessments of the health needs of children and young people to determine eligibility for CCC funding.
* Coordinating and completing MDT assessments and reviews in accordance with the National Framework.
* Provide a high standard of holistic health assessments within the agreed timeframes.
* Commissioning of the packages of care to meet the unmet health needs of children and young people eligible for CCC.
* Supporting children and young people with the offer of a personal health budget to meet the health outcomes.
* Case management for children and young people eligible for funding via CCC.
* Record keeping as per NENC policy and procedures.
* Working in partnership with children, young people, and their representatives.
* Building and maintaining relationships with Local Authorities, providers, Foundation Trusts, and the voluntary sector.
* Contributing to the ICB's achievement of statutory duties and supporting the transition of young people to adulthood.
* Contributing to the ICB statutory duties for SEND.

Job Description

Person Specification
Qualifications
* Essential: First Level Registration NMC - RSCN, degree or relevant experience
* Desirable: Qualification in children's health care or Learning Disabilities
Experience
* Essential: Experience of clinical audit; experience of completing multi-agency assessments; previous experience of working within a busy office environment; experience in a multi-disciplinary team setting.
* Desirable: Prior experience in Continuing Care.
