Job Summary

Due to retirement, an exciting opportunity has arisen to join our friendly, dedicated team at Glebe House Surgery. We are looking to recruit an experienced Practice nurse to join our nursing team.

Hours available, up to 37 hours, must be available to work until 6.30 on some shifts.

Salary is competitive, negotiable, dependent upon experience and will be discussed at interview. NHS pension provided.

Main duties of the job

We are a busy, friendly, forward-thinking team dedicated to delivering high-quality care to our patients in our state-of-the-art facilities. We provide a wide range of services including general medical services and enhanced services such as contraceptive services and extended opening hours. We provide a high-quality learning environment and are committed to evidence-based patient care. We are a teaching practice for GP registrars, medical students, and support our nurses in advancing their practice.


About Us

Glebe House Surgery is a single-site practice based in the semi-rural market town of Bedale at the gateway to the Yorkshire Dales. We are a friendly, supportive practice with a strong desire to develop our services and further expand upon our existing skills and clinical interests.

We achieve highly in QOF thanks to our Nursing team's dedication and received a rating of 'good' in our latest CQC inspection.

Our nursing team includes 4 Practice nurses, 3 healthcare assistants, and phlebotomy staff. We work together to provide treatment room sessions, chronic disease management including Diabetes and Respiratory. Experience in child and adult immunisations, dressings and wound care, cervical smear taking, and chronic disease management would be desirable. Full training and mentorship will be provided for the right candidate.


Job Responsibilities

Hours: Variable hours available up to 37 hours

Reports to: Practice Manager

Accountable (non-clinical) to: Practice Manager

Accountable Professionally: GP Partners / NMC

The Post Holder Will:

* Assess, plan, develop, implement, and evaluate programmes to promote health and well-being of patients attending the practice.
* Deliver clinical care within the treatment room setting of (subject to completion of training):
* Wound care acute, post-operative, chronic, and minor injuries.
* Complex wound management involving Doppler assessment and compression bandaging.
* Child and adult immunisations and the annual influenza / Covid immunisation programmes.
* Sexual health services.
* Cervical screening.
* Undertake ECGs.
* Venepuncture.
* Administer prescribed medication for shared care agreements (amber drugs) in line with National, Local, and NMC code of conduct.
* Work with patients to support compliance and adherence to treatments, providing information on medication regimes, side-effects, and interactions.
* Comply with all infection prevention and control standards.
* Assist with minor surgical procedures.
* Support health promotion and education.
* Assist in screening for patients at risk of long-term conditions and undertake health risk assessments, providing advice and making referrals where appropriate.
* Identify and refer patients with mental health needs.
* Assist in chronic disease management, including regular calibration and routine checks as per Spirometry guidelines (after training).
* Following advanced training, undertake assessment and management of patients with long-term conditions like diabetes, stroke, hypertension, COPD, and asthma.
* Deliver immunisations under PSD or PGD for travel health assessments and provide advice after completing necessary study and qualifications.
* Undertake collection, storage, and dispatch of pathological specimens, managing results and providing patient information.


Risk Management

* Ensure safe storage, rotation, and disposal of vaccines and drugs, applying infection control standards.
* Maintain NMC registration and work within the professional code of conduct.
* Report faults, repairs, or incidents to the Practice Manager.
* Check patient identification before procedures and ensure safe handling of patients.
* Promote health and safety in the workplace.
* Offer and act as a chaperone for intimate examinations.


Managing Information

* Maximise opportunities to meet QOF objectives.
* Document clinical information using practice templates.
* Assist in clinical audits and record-keeping for procedures, cytology, immunisations, and drug monitoring.


Communication

* Treat all patients and relatives with respect and dignity.
* Use effective communication styles to ensure informed consent.
* Recognise and adapt to different communication needs based on understanding, culture, and preferences.
* Act as an advocate for patients and colleagues.
* Maintain accurate and confidential clinical records.
* Participate in practice meetings.


Learning and Development

* Participate in annual performance reviews and maintain training records.
* Identify and address training needs.
* Update CPD logs regularly for revalidation.
* Engage in self-directed learning and training, and assist in teaching students.
* Maintain professional registration and participate in courses and study days.
* Complete mandatory training on time.
* Mentor student nurses and junior staff as required.
* Stay updated with local and national healthcare developments.


Service Improvement

* Participate in research and patient surveys.
* Engage in clinical audits and service development initiatives.


Quality Service Delivery

* Work within own competence and professional standards.
* Identify and communicate quality and risk issues.
* Reflect on own performance and contribute to team effectiveness.
* Work with other agencies to meet patient needs.
* Prioritise and manage workload effectively.
* Maintain comprehensive and accurate patient records.
* Provide evidence-based, individualised patient care.
* Follow policies on safeguarding and vulnerable adults/children.

* Handle all sensitive information responsibly.
