Senior Nurse – Community (Long Term Conditions)
We are looking for a Senior Nurse experienced in working with patients in the community who have complex long‑term conditions. You will join our Liskeard and Looe Community Matron Team for a secondment or fixed‑term contract, working closely with Adult Community Services, Adult Social Care and Primary Care to enable patients to remain independent at home and reduce avoidable hospital admissions.
Core Responsibilities
* Autonomous Clinical Practice: Take a comprehensive history, perform a systematic physical examination, and establish baseline data to inform personalised care plans.
* Integrate care across all settings; prevent duplication, fragmentation and delays as patients move between care environments.
* Develop therapeutic relationships through active listening, checking understanding, and engaging patients and carers.
* Education: Provide knowledge, empower patients and families to make informed decisions, mentor students, and develop education for colleagues.
* Leadership/Management: Contribute to the development, implementation and monitoring of policies, procedures and guidelines.
* Research: Develop a special area of expertise, articulate contemporary evidence and approaches to practice and management.
Qualifications & Skills
* Completed the HEAB 358 Advancing Practice in Long Term Conditions Management module or equivalent.
* Independent prescriber or working towards the qualification.
* High level of communication, problem‑solving and decision‑making skills.
